Plan of Care

At the time of a new resident’s admission to the home, we work with the resident and/or their friend, relative or representative if appropriate, to gain information and draw up a written plan of the care we aim to provide. The plan sets out care objectives and incorporates any necessary risk assessments.

In addition to the regular monitoring of the plan on a day-to-day basis, the home will arrange organised reviews at least monthly or as needs change. The care coordinator or the designated senior care assistant, who will sign the review accordingly, will carry out these reviews.

At least twice a year, and as necessary, a formal review involving at least the resident and care manager will discuss the progress of the care plan. This review will be signed by the resident and/or their close relative or representative, and the care manager.

Every resident has access to their plan and is encouraged to participate as fully as possible in the care planning process.
